NBME 12 Prediction Direction: Insufficient Community Data

Community score-pair data for NBME 12 is limited and mixed — some students score higher on the real exam than NBME 12 predicted, others score lower. No reliable directional bias estimate exists for NBME 12. The current Step 2 CK passing threshold is 218 (effective July 1, 2025). Do not use NBME 12 as a scheduling signal — use NBME 13 or UWSA 2 for that decision. No peer-reviewed predictive accuracy study exists for any Step 2 CK practice assessment (unlike Step 1's Seal et al. 2020, Cureus, PMC7198101).

Why NBME 12 Belongs in Your Step 2 CK Week 5–6 Plan

NBME 12 serves a specific function in Step 2 CK preparation that no other form fills: it is the last structured progress benchmark before the final preparation sprint. At Week 5–6, you have completed the bulk of your question bank and content review but still have time to identify and address knowledge gaps. NBME 12's score is meaningful — it tells you whether your preparation trajectory is on track for NBME 13 (Week 6) and, ultimately, your exam date. Unlike NBME 9–11, which are better suited for earlier self-assessment, NBME 12 sits close enough to exam day that its predicted score carries real scheduling relevance.

NBME 12 is rated moderate difficulty — distinct from NBME 13, 14, and 15, which are all rated hard. This matters in two ways. First, your NBME 12 predicted score may read slightly higher than what NBME 13 will show for the same level of preparation, simply because the questions are less demanding. Expect a potential drop of 3–7 points when you move from NBME 12 to NBME 13. Second, the moderate difficulty means NBME 12 still rewards breadth of knowledge over nuanced reasoning — which is appropriate for Week 5–6 when you should be consolidating core material, not fine-tuning edge-case reasoning.

Difficulty rating and timing reflect scoreData project data. Prediction direction for NBME 12 is not reliably established — community data is insufficient and mixed. No peer-reviewed predictive accuracy study exists for any Step 2 CK practice assessment (unlike Step 1's Seal et al. 2020, Cureus, PMC7198101).

Formula: 300.18 − 1.09 × wrong answers. Passing threshold: 218 (USMLE.org, effective July 1, 2025).

240+ competitive threshold corresponds to 55 or fewer wrong answers (wrong=55 → 240.23; wrong=56 → 239.14). Specialty thresholds (230+/240+) reflect community consensus — not official NRMP data. NBME 12 is moderate difficulty; community data on prediction direction is insufficient — treat predicted score as approximate and do not use it as a scheduling signal.

Community consensus data. NBME 12's moderate difficulty distinguishes it from NBME 13+ (hard). Because NBME 13 is harder, most students get more questions wrong on it — producing a lower predicted score than NBME 12 even at the same knowledge level. At 60 wrong answers: NBME 11 → ~235, NBME 12 → ~235, NBME 13 → ~235, NBME 14 → ~234.
